Abstract

Computerize implementation of the system in the health sector aims to processing data easier for users to store data in, manage the data and get information from the collection of data that has been processed. Clinic Gita Medika Palembang is one of the many businesses that move in the field of health services that manage and present data are not using computerize system, giving rise to difficulties and problems when data is required (such as: data search, data store and data report). At this research will build a software application for physician practices perform data processing at the Clinic Gita Medika Palembang. Waterfall model used in engineering software. At the stage of system analysis is performed to analyze the running system, creates business processes for the running system, concluded the problems encountered, to provide an alternative problem-solving, analyzing the needs of the system with access control lists, functional requirements, and analyze the proposed system modeling system such as: Context Diagram, Data Flow Diagram, Entity Relationship Diagram and Data Dictionary. System developments are creates a data structure and architectural design such as design of menu structure and interface design of form. The programming language was PHP versions 4.1.1 with MySQL versions 1.3 as database.
