Abstract

World Health Organization (WHO) reported that the number of Maternal Mortality Rate (MMR) reaches 163/100,000 Live Births (LB). In 2018, The Public Health Department of Palembang confirmed that the number of MMR is 4 cases and the number of Infant Mortality Rate (IMR) is 24 cases in 26,837 LB. Midwifery caring of Continuity of Care (COC) is a sustainable service to mother during the pregnancy, labor, and postpartum which aims to prevent the complication risk during the pregnancy, labor, and postpartum and to decrease the number of MMR and IMR. This final report provided midwifery caring of COC to Mrs. “Y” at Midwife Lismarini’s Independent Midwifery Practice from October 16, 2021 to February 1, 2022. The COC was conducted from pregnancy, labor, newborn baby, postpartum and neonate. Primary and secondary data were used to collect the data. This study case used Subject, Object, Assessment, and Plan (SOAP) note. The result of COC showed that from the pregnancy, labor, newborn baby, postpartum, to neonate was in physiological scope. In short, the pregnancy, labor, and newborn baby were in normal condition. Meanwhile, the postpartum and neonate was in physiological scope. The Independent Midwifery Practice is suggested to maintain the quality of midwifery caring service.
