Abstract

Based on the data from the World Health Organization (WHO), the Maternal Mortality Rate in the world, in 2018, was 831 per 100.000 Live Birth. Meanwhile, the Infant Mortality Rate in the world, in 2017, was 29 per 1000 Live Birth. In Indonesia, the Maternal Mortality Rate was estimated to be 305 per 100.000 Live Birth in 2015. The Health Department of Palembang city stated that the Mother Mortality Rate in 2018 was 4 persons from 26.837 Live Birth, while the Infant Mortality Rate was 24 per 26.837 Live Birth, consisting of 18 babies in the neonate (0 to 28 days old) and 6 babies (29 days to 11 months old). One of the ways to decrease both Maternal Mortality Rate and Infant Mortality Rate is giving Midwifery Continuity of Care. This final project report aims to give Midwifery Continuity of Care for Mrs. “D” at Ellan’s Midwifery Independent Practice in Palembang, starting from the pregnancy, childbirth, new-born baby phase, postpartum period, and neonate. This project used the descriptive method. The data were gathered from the primary data and by observing the secondary data, which were documented by using the SOAP method, conducted from February 26 to April 3, 2021. The result from the treatment given to Mrs. “D” showed that the pregnancy, childbirth, new born baby phase, the 6-hour postpartum and the 5-day postpartum period, and 5-day neonatewere in a normal limit/diagnosis and still inside the physiological scope. The conclusion suggests that giving Midwifery Continuity of Care allowed the mother’s and baby’s conditions to be monitored, and the emerging problems were able to be detected earlier.
