Abstract
World Health Organization (WHO) data shows that the Maternal Mortality Rate (MMR) is still high, around 295.000. In Indonesia, maternal mortality increased by 300 cases in 2019 to around 4.400 deaths in 2020. The Infant Mortality Rate (IMR) increased by almost 40 percent to 44.000 cases in 2020. As one of the efforts to tackle MMR and IMR, it is necessary midwifery care Continuity of Care. This Final Project Report provides midwifery care Continuity of Care to Mrs “A” aged 20 years in the Independent Practice of Midwife Lismarini Palembang starting from pregnancy, childbirth, newborn, postpartum, neonates and family planning. Data collection techniques using primary data and secondary data. The case study method used is using SOAP records of progress carried out from 21 October 2021- 24 February 2022. The results obtained in Mrs “A” starting from the period of pregnancy, childbirth, newborn, postpartum, neonates and family planning within normal limits. Conclusion during pregnancy the mother experienced a low-lying placenta but 1 week later the placenta was in a normal position, normal delivery, normal newborn, neonates, postpasrtum and normal family planning, the condition of the mother and baby were still within the physiological scope. Suggestions for the next author to improve self-skills in providing Continuity of Care midwifery care services in the laboratory before going to clinical practice so that midwifery care Continuity of Care is of quality in completing the final project report.
