Abstract

Death on the world in the 2017 amounted to 810 woman dies every day with 80% of deaths caused by heavy bleeding, childbirth infections, preeclampsia and eclampsia, and unsafe abortions. In 2016, the number of neonatal deaths in the world amounted to 19 from 1000 live births, one of the efforts to reduce AKI and AKB were made into continuity of care. The method used on writing this final task reports is the method descriptive with data collection technique that are primary data and secondary data aldo documenting method using SOAP administered. The results of the obstetrics orphanage report referred to Mrs “D”’ at home childbirth in to the maternity hospital Mitra Ananda Palembang had not found any pathological problem and still within normal limits. Conclusion care of continuity of care to the performed on Mrs “D” in the correct stages, starting pregnancy of childbirth, baby was born, puerperal, neonatus, contraceptive acceptor. Implementation, assesments, evaluation is done according to the needs of the mother and her baby.
