Abstract

The Data from World Health Organization (WHO) about the Maternal Mortality Rate is one of the indicators to know the success of health department efforts. It was estimated that in 2017 the Maternal Mortality Rate in the world was 211 per 100.000 Live Birth and the Infant Mortality Rate was 18 per 1000 Live Birth (KH). One of the efforts to decrease the Maternal Mortality Rate and Infant Mortality Rate is by giving midwifery care for Continuity of Care (COC). The aim of this final project report is to enable the researcher to give a midwifery Continuity of Care to Ms “N”, starting from pregnancy, childbirth, newborn baby, postpartum period, up to family planning program, at Ellna’s independent midwifery practice. This final project report used a case study design about midwifery care named Continuity of Care (COC) and was documented by using SOAP (Subjective, Objective, Assessment, and Plan) from Ms “N” during the midwifery independent practice of Ellna, from March 01 to Mei 02, 2021, starting from the pregnancy, childbirth, new-born baby, postpartum period, up to neonate. The data was gathered from the primary and secondary data sources. The result showed that Mrs. “N” case starting from the pregnancy with numb hands, childbirth, newborn baby, postpartum period, up to neonate was still at the normal limit and inside the physiological scope. It is concluded that the midwifery caring method, named Continuity of Care (COC), implemented to Mrs. “N” by using SOAP, resulted in the baby was able to be monitored properly and the complications were able to be detected early. For the suggestion, it is suggested that the patient who will give birth should maintain both the mother’s and baby’s health.
