Abstract
The World Health Organization (WHO) stated the world Maternal Mortality Rate in 2017 was 462 per 10.000 Live Birth. According to SDKI, the Maternal Mortality Rate in Indonesia was still high in the year of 2017, which was 305 per 100.000 Live Birth, while it was 7 per 27.876 Live Birth in Palembang. One of the efforts to decrease the Maternal Mortality Rate and Infant Mortality Rate is implementing the Midwifery Continuity of Care (COC), which is a continuous care for the pregnant woman starting from the pregnancy up to the family planning as the effort for decreasing the Maternal Mortality Rate and Infant Mortality Rate. The aim of this final project report is to enable the researcher to give the Midwifery Continuity of Care (COC) to Mrs. “Z” starting from pregnancy, child-birth, new-born baby, postpartum up to neonate during the Josephine’s Midwifery Independent Practice. This final project report was being able to give a midwifery caring using Continuity of Care (COC) approach to Mrs. “Z” at Josephine’s Midwife Independent Practice since March 02, 2021, including pregnancy, child-birth, new-born baby, postpartum, neonate, and Family Planning. The data was gathered from the primary and secondary data. The implemented midwifery caring was the Continuity of Care approach using SOAP (Subjective, Objective, Assessment, and Plan) method. The result of the midwifery Continuity of Care (COC) for Mrs. “Z” showed that the caring involved pregnancy phase I to IV, child-birth, new-born baby, postpartum period, and neonate. The conclusion based on analysing gathered data from Mrs. “Z” suggested that the pregnancy diagnosis, child-birth, new-born baby, postpartum period, and neonate were normal.
