Abstract
In 2017, World Health Organization (WHO) reported that the Maternal Mortality Rate (MMR) in the world was 462/10,000 Live Birth (LB). Meanwhile, in Indonesia, the MMR was 305/100,000 LB as reported by the Indonesian Nursing Diagnosis Standard (INDS). In addition, the MMR was 7/27,876 LB in Palembang. One of the ways to decrease the MMR and IMR was conducting midwifery caring of Continuity of Care (COC). It is continuously provided from pregnancy to labor. This final task report aimed to provide midwifery caring of COC from pregnancy, labor, newborn baby (NB), postpartum, neonates, and family planning (FP) at Hadawiyah Independence Midwifery Practice. This final assignment provided midwifery caring of COC to Mrs. “N” at Hadawiyah Independence Midwifery Practice since March 8, 2021, from pregnancy, labor, newborn baby (NB), postpartum, neonate, and family planning (FP). The data collection technique used primary and secondary data. The midwifery method employed midwifery caring of COC approach and SOAP method. The result of midwifery caring of COC to Mrs. “N” in the pregnancy, labor phase I-IV, newborn baby (NB), postpartum, neonate, and family planning (FP) was found in physiological scope. The conclusion of the data based on the data assessment of Mrs. “N” showed that the assessment of pregnancy, labor, newborn baby (NB), postpartum, neonate, and family planning (FP) assessments were normal. Planning and evaluation of midwifery caring of COC to Mrs. “N” during pregnancy, labor, newborn baby (NB), postpartum, neonate, and family planning (FP) had been done based on the assessment. It is suggested to the patient to keep her health, self-hygiene, and baby hygiene.
