Abstract

CV. Anugerah Utama is a business entity that produces and sells building materials. The main problem, there are often many defect bricks. Every month the average production defect reaches 0.005%-0.01%, this affects the quality. The research aims to identify the causes of defects, corrective measures, and implementation of statistical process control methods. In the study, found 4 types of defects, namely porosity, porous, cracks, and dimensions that do not match. The SPC method utilizes quality control tools to solve problems, such as check sheets, pareto, flowcharts, histograms, p-charts, and fishbone diagrams. The application of 5-why analysis and 5W+1H using brainstorming succeeded in finding various causes of defects consisting of 5 factors, man, machines, materials, methods, and environment. Improvements were made, namely checking materials, providing coffee facilities, free patching, first aid kits, routine briefings, training, and socialization of K3. Then, perform machine maintenance, such as descaling cement mixer, lubricating the lever, replacing the out-of-print mold, and checking the base. Then, pay attention to the condition of the stacking blocks, fill the waterlogged surface, and make a soil rake to smooth the surface. Repairs were carried out in April 2021, producing good results, the total percentage of defects decreased from 0.65% to 0.34%. Then, the DPMO (Defects Per Million Opportunities) indicator used shows that the DPMO value has decreased from 897 DPMO to 485 DPMO, meaning that the quality is better and competitive.
