Abstract
Background : Discharge planning is an effort that can be done by nurses in providing care to clients. Discharge planning can provide benefits such as improving the quality of life of clients, making proper care days, and reducing the number of readmission. Discharge planning must be carried out from the start of the client's entry, and continue until the client is sent home. Nurses have an important role in implementing discharge planning. Nurses who understand the methods and components of discharge planning will be the starting point for implementing discharge planning properly. A preliminary study at Myria Hospital from the January-May discharge planning audit results is still very low, so the causes, constraints, and opportunities for improvement need to be known in order to carry out better discharge planning. Objective : This study aims to determine the description of the implementation of discharge planning nurses. Method : This research method uses descriptive analytic survey with cross sectional approach. Sampling uses total sampling technique with a total sample of 30 respondents. The discharge planning implementation was assessed using a questionnaire that had been tested for validity and reliability. Results : The results of the univariate analysis showed the implementation of discharge planning in a good category of 14 (46.7%), a fairly good category of 15 (50%), and an unfavorable category of 1 (3.3%). Based on the results of this study, training on discharge planning, revision of the forms used, revisions to the discharge planning audit instrument were needed to improve better implementation for all disciplines.
