Abstract
Background: Creatinine is a clinical chemistry parameter used to assess kidney func-tion. Creatinine testing typically uses serum, but requires a clotting period before pro-cessing. Lithium heparin plasma can be used as an alternative specimen because it does not require clotting, thereby speeding up the testing process. Differences in the composition of the two specimens are thought to influence creatinine test results. Objective: This study aims to conduct a comparative evaluation of creatinine levels in Serum Separator Tube (SST) serum samples and lithium heparin plasma samples using an enzymatic method. Methods: This analytical observational study using a cross-sectional design was con-ducted at the Palembang Center for Public Health Laboratories (BBLKM) in July 2026. The sample consisted of 35 participants selected using total sampling. Creatinine levels were measured using the enzymatic kinetic method with the BioSystem BA200. Data analysis included the Shapiro-Wilk test, descriptive analysis, and the paired-sample t-test. Results: The mean serum creatinine level in the SST group was 0.9617 mg/dL, and the mean plasma lithium heparin level was 0.9846 mg/dL. The results of the paired-sample t-test showed a p-value of 0.130 (p > 0.05). Conclusion: There was no difference in the results of creatinine level tests between serum and lithium heparin plasma samples. Recommendation: Heparinized lithium plasma is considered an alternative specimen for enzymatic creatinine testing; further research is needed using a variety of specimens, methods, and subject characteristics.
