Abstract

Midwifery care during pregnancy, labor, newborn care, postpartum, and the neonatal period represents a critical service in enhancing maternal and infant health status and reducing morbidity and mortality rates. Midwifery Continuity of Care (CoC) is defined as a healthcare model that provides uninterrupted and comprehensive care throughout the continuum of childbearing, from pregnancy through the neonatal phase. This final report aims to deliver comprehensive and sustainable Midwifery Continuity of Care for Mrs. “R” at Midwife “E” Independent Practice in Palembang. The care was implemented using a midwifery management approach and SOAP (Subjective, Objective, Assessment, and Plan) documentation. Data collection methods included interviews, clinical observations, physical examinations, midwifery assessments, and diagnostic investigations. The scope of care spanned from the third trimester of pregnancy through the neonatal period. Clinical outcomes indicated a complication-free pregnancy, a normal delivery resulting in a healthy newborn, and physiological progression during the postpartum and neonatal periods without any identified morbidities. Implementing Midwifery Continuity of Care facilitates optimal health monitoring for mothers and infants, enables the early detection of risk factors or complications, and elevates the overall quality of midwifery services. In conclusion, the Midwifery Continuity of Care provided to Mrs. “R” was successfully executed in accordance with midwifery clinical standards, resulting in favorable health outcomes for both mother and infant. It is recommended that the CoC model be consistently applied to ensure the ongoing enhancement of comprehensive and continuous midwifery service quality.
