Abstract

Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) remain health problems that require preventative measures through continuous midwifery services. One such effort is the implementation of Continuity of Care (CoC) to detect and prevent complications early. Care for Mrs. "A," aged 23 years (G2P1A0), was provided from August 10, 2025, to April 25, 2026, starting from the first trimester of pregnancy, the postpartum period, and the neonatal period. Data collection techniques included interviews, observations, physical examinations, supporting examinations, literature review, and documentation recorded using the SOAP method. Throughout the pregnancy, the mother and fetus were in good condition. During labor, the fetus was found to be in a breech position, so the mother was referred to the hospital and delivered by cesarean section (CS). During the postpartum period, the mother's condition was monitored, surgical wound care was provided, and health education was provided. The infant was monitored throughout the neonatal period, and no complications were found. The implementation of Continuity of Care midwifery care with the Charitas spirituality approach was successful, ensuring the health of the mother and baby until the end of the follow-up period. It is hoped that healthcare workers and students will continue to implement continuous midwifery care according to service standards and integrate Charitas spirituality values to improve the quality of maternal and infant care.
