Abstract

The World Health Organization (WHO) indicates that the global Maternal Mortality Rate (MMR) reached 260,000 deaths in 2023. In Indonesia, the 2020 MMR was recorded at 189 per 100,000 live births, while 2024 data for Palembang City reported 15 maternal fatalities and 119 infant deaths. This case study aimed to provide comprehensive Continuity of Care (CoC) for Mrs. “C” to facilitate early detection of complications from the antenatal to the neonatal periods. Utilizing a case study design, data were collected through primary and secondary sources and documented using the SOAP (Subjective, Objective, Assessment, and Plan) method. The care was administered from September 28, 2025, to April 22, 2026, at Midwife “L” Independent Practice Palembang. Clinical findings during the first and second trimesters indicated a physiological pregnancy. However, in the third trimester, early detection identified facial and lower limb edema accompanied by a blood pressure of 150/90 mmHg, necessitating a referral to the hospital. Consequently, delivery was conducted via Caesarean Section. During the postpartum period, the mother showed a positive recovery with normal uterine involution, appropriate lochia stages, a clean and dry surgical wound, and effective lactation. The new-born was delivered full-term (2,900 grams, 48 cm) with strong reflexes, normal vital signs, and no complications. The study concludes that the CoC model was effectively implemented according to service standards, allowing for the timely detection of risk factors and ensuring the well-being of both mother and infant. It is recommended that midwives maintain and enhance the quality of midwifery services provided to clients.
