Abstract
The persistent challenge of maternal and infant mortality remains a primary focus of global public health, serving as a critical barometer for the quality and accessibility of obstetric and neonatal care systems. In the Indonesian context, maternal mortality saw a significant recorded decrease from 34,266 deaths in 2023 to 4,151 in 2024, with the Maternal Mortality Rate (MMR) declining from 390 to 189 per 100,000 live births between 1991 and 2020, nearly achieving the national target of 183. Concurrently, infant mortality declined from 34,266 deaths in 2023 to 33,131 in 2024, although these figures have yet to meet designated national objectives. This case study evaluates the implementation of the Midwifery Continuity of Care (CoC) model for Mrs. "L," encompassing the periods of pregnancy, labor, newborn, neonatal, and postpartum care, integrated with Charitas spiritual values at Midwifery ”L” Independent Practice. Data were collected through primary methods, including anamnesis, physical examinations, and direct observation, as well as secondary medical record reviews, with all interventions documented using the SOAP (Subjective, Objective, Assessment, Plan) framework. Clinical findings at the initial first-trimester assessment (7–8 weeks gestation) revealed moderate anemia and Chronic Energy Deficiency (CED); however, through sustained clinical intervention, the patient’s condition improved to mild anemia by the third trimester (34–35 weeks). Labor occurred at term and progressed normally, followed by healthy newborn, neonatal, and postpartum periods within physiological limits. The positive clinical trajectory from the first to the third trimester underscores the efficacy of the CoC model in optimizing maternal health outcomes. Consequently, it is recommended that midwifery practitioners maintain and enhance service quality by intensifying professional counseling for clients and their families to ensure sustained maternal and neonatal well-being.
