Abstract

Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) continue to serve as key indicators of population health and healthcare system performance. In South Sumatra Province, the MMR was reported at 64 per 100,000 live births in 2022, while the IMR reached 4.3 per 1,000 live births in 2023. These figures underscore the persistent need to strengthen maternal and neonatal healthcare services through comprehensive and continuous care. The Continuity of Care (CoC) model is designed to improve the quality of midwifery services by ensuring seamless, woman-centered care across the continuum of pregnancy, childbirth, postpartum, and neonatal periods, while facilitating the early identification and management of potential complications. Accordingly, this case study aimed to implement comprehensive midwifery Continuity of Care services grounded in the principles of Charitas spirituality. This study employed a case study design conducted at Midwifery “L” Independent Practice, Palembang, involving Mrs. “I”, a 20-year-old woman with obstetric status G2P1A0 at 12–13 weeks of gestation. Midwifery care was documented using the Subjective, Objective, Assessment, and Plan (SOAP) format. Data were collected through interviews, observations, physical examinations, supporting examinations, documentation review, and literature review. The pregnancy progressed within normal physiological parameters, with both maternal and fetal conditions remaining stable and free from obstetric complications. A spontaneous vaginal delivery occurred at 39–40 weeks of gestation, resulting in the birth of a healthy live female infant. During the intrapartum period, amniotomy was performed at near-complete cervical dilatation without any associated maternal or neonatal complications. The postpartum course was uneventful and characterized by normal uterine involution and satisfactory perineal wound healing. Furthermore, the newborn exhibited normal growth and age-appropriate physiological adaptation throughout the neonatal period. Based on the results of the care provided, the implementation of midwifery Continuity of Care was successfully carried out in a comprehensive, continuous, and holistic manner from pregnancy through the neonatal period. This approach supported maternal and neonatal health monitoring as well as the early detection of potential complications. It is expected that the implementation of Continuity of Care will continue to be optimized by midwives with the support of mothers and their families in order to enhance the quality of healthcare services and improve maternal and neonatal
