Abstract

Maternal and Infant Mortality Rates (MMR and IMR) remain critical public health challenges at both global and national levels. In 2023, the WHO reported approxi-mately 260,000 maternal deaths, following 2.3 million neonatal fatalities recorded in 2022. A primary strategy to mitigate these rates is the implementation of Midwifery Continuity of Care (CoC), which provides integrated services spanning the antena-tal, intranatal, postnatal, and neonatal periods. This study aimed to provide compre-hensive CoC for Mrs. “Y” at Midwife “E” Independent Practice Palembang, un-derpinned by Charitas spirituality. Adopting a case study design, this study utilized the SOAP (Subjective, Objective, Assessment, and Plan) documentation method from July 21, 2025, to March 6, 2026. Clinical findings indicated that while Mrs. “Y” experienced physiological symptoms such as emesis gravidarum and dental pain during the first trimester, as well as general discomfort in the third trimester, no pathological complications were observed. Labor progressed normally, and both the postnatal and neonatal outcomes remained within physiological limits. Furthermore, the integration of Charitas spirituality throughout the care process provided signifi-cant emotional support and enhanced maternal confidence from pregnancy through the postpartum period. The study concludes that the CoC model was successfully implemented for Mrs. “Y,” resulting in healthy physiological outcomes without complications. It is recommended that the application of holistic CoC services be further enhanced to improve the overall quality of maternal and child health out-comes.
