Abstract

Background : Asthma, characterized by shortness of breath and decreased oxygen saturation, affects approximately 300 million people worldwide, with a mortality rate reaching 1,000 per day. Non-pharmacological interventions such as adjusting body position are necessary to assist breathing. The semi-Fowler's position is known to increase lung expansion and improve ventilation, but research on its effect on oxygen saturation and respiratory rate in asthma patients in the emergency department (ER) is limited. Objective : Analyze the differences in oxygen saturation and respiratory rate before and after the application of the semifowler position in asthma patients at the Emergency Department of RSUD Siti Fatimah, South Sumatra Province. Methods : This study used a pre-experimental design with a one-group pretest–posttest approach. The sample was selected using a purposive sampling technique. Oxygen saturation was measured using a pulse oximeter, while respiratory rate was measured through direct observation before and after the semi-Fowler's position intervention. Data were analyzed to determine differences in values before and after the intervention. Result : The univariate and bivariate results of respondent characteristics were age, gender, education, duration of suffering, and history of drug use. The results of the Two-Sided t-test showed p <0.001 for oxygen saturation less than (p <0.05). The results of the Two-Sided t-test showed p <0.001 for respiratory frequency less than (p <0.05). Conclusion : There were significant differences in oxygen saturation and respiratory rate before and after the application of the semifowler position in asthma patients. The semifowler position can be used as an effective non-pharmacological nursing intervention to improve oxygenation and reduce respiratory effort in asthma patients in emergency care. Suggestion : The semifowler position is recommended to be routinely applied as an independent nursing intervention in the management of asthma patients in the Emergency Department and as a reference for future research.
