Abstract
Complications during pregnancy, childbirth, the postpartum period, and the neonatal phase significantly contribute to elevated maternal and infant mortality rates, both of which remain relatively high in Indonesia. According to data from the World Health Organization, the global maternal mortality rate (MMR) in 2021 reached 280,000 cases, while the infant mortality rate (IMR) was approximately 1.4 million. In Indonesia, the MMR for the same year was 305 per 100,000 live births, and the IMR stood at 22 per 1,000 live births. In South Sumatra, the 2023 MMR was reported at 105 cases, with an IMR of 2.4 per 1,000 live births. Meanwhile, in Palembang City, the MMR for both 2023 and 2024 totaled 53 cases, and the IMR was recorded at 3 per 1,000 live births. These statistics underscore the critical need for the implementation of midwifery care through the Continuity of Care (CoC) model as a strategic approach to reducing both maternal and infant mortality rates. This report aims to provide midwifery Continuity of Care for Mrs. "R" during pregnancy, childbirth, postpartum, and the neonatal period. This case study was conducted at the Midwife "K" Independent Practice in Palembang from June 21, 2024, to January 15, 2025, using both primary and secondary data collection techniques, based on a midwifery management approach under CHARITAS values. The findings indicated that from the initial antenatal visit through the third trimester, Mrs. "R" experienced normal physiological conditions, including leucorrhea within normal limits. Labor progressed without complications, resulting in the birth of a healthy female infant weighing 3,200 grams and measuring 49 centimeters in length. Both the postpartum and neonatal periods proceeded smoothly, with no observed complications. The researcher concludes that the midwifery care provided to Mrs. "R", including antenatal care for a full-term pregnancy, childbirth, postpartum, and neonatal care, was within normal parameters. It is recommended that the clinical setting maintain the current quality of midwifery services.
