Abstract

According to the WHO, the maternal mortality rate in 2023 was very high, with approximately 260,000 women dying during pregnancy and childbirth. In 2022, the infant mortality rate in several ASEAN countries ranged from 0.7 to 39.4 deaths per 1,000 live births. In Indonesia, according to information from the Ministry of Health, the maternal mortality rate (MMR) in 2022 reached 230 per 100,000 live births. In Indonesia, the infant mortality rate (IMR) in 2022 based on BPS data was around 23.5 deaths per 1000 live births. Referring to the data from the Health Office of South Sumatra Province, in 2019 there were 105 maternal deaths recorded. Furthermore, according to the Health Office, in 2022, the cause of the highest infant mortality rate was asphyxia, namely 152 cases (35%). In 2020, the maternal mortality rate in the city of Palembang was 14 people. The purpose of this report is one of the efforts to reduce MMR and IMR by providing midwifery care, namely Continuity of Care (COC), for Mrs. "N" based on the spirituality of Charitas. This final assignment’s report was carried out from June 27, 2024 to March 11, 2025, starting from pregnancy, childbirth, newborns, postpartum, and neonates. This case study method used the SOAP approach, which includes subjective, objective, assessment, and plan data. The results of the study indicate that during pregnancy and postpartum, and newborn, its condition were physiological. However, during the first phase of childbirth, a referral was made to "P" Hospital with a diagnosis of PPROM. The recommendation is for PMB to maintain the midwifery care services, that is, Continuity of Care.
