Abstract

The World Health Organization (WHO) reported that in 2023, maternal mortality reached 260,000 women globally, while in 2022, neonatal mortality reached 2.3 million children. In Indonesia, the Maternal Mortality Rate (MMR) over the past four years was recorded at 189 per 100,000 live births, and the Neonatal Mortality Rate (NMR) at 18.6 per 1,000 live births. In South Sumatra Province, there were 107 maternal deaths and 656 neonatal deaths in. In Palembang City, 18 maternal and approximately 18 neonatal deaths were recorded in 2020. This report aims to describe the provision of midwifery care based on the 2024Continuity of Care (CoC) model for Mrs. "R" during the pregnancy, childbirth, newborn, postpartum and neonates periods, using a Charitas spirituality approach. This descriptive case study employed a CoC care model and utilized the SOAP (Subjective, Objective, Analysis and Planning) documentation format to record the pregnancy, childbirth, newborn, postpartum and neonates care of Mrs. "R." The case study was conducted from July 3, 2024, to March 15, 2025. Both primary and secondary data were collected directly from Mrs. "R." The results showed that the first and second trimesters of pregnancy were within normal limits; however, in the third trimester, Mrs. "R" was diagnosed with moderate anemia (Hb 8.0 g/dL). Labor occurred at term, with normal outcomes for the baby at birth (BBL), postpartum period (puerperium), and neonatal period. In conclusion, Mrs. "R" was cooperative during subjective data collection. Objectively, in the third trimester, she experienced moderate anemia. The diagnosis, care plan, and implementation were carried out according to midwifery standards, integrating the values of Spiritual Charitas. It is recommended that midwives continue to maintain and enhance the quality of midwifery care services.
