Abstract
World Health Organization (WHO) reported that the Maternal Mortality Rate (MMR) in 2020 287,000 people worldwide. It is in line with the data of Indonesian Ministry of Health, which found that in the same year, the MMR reached 189 per 100,000 live births. Two years later, in 2022, the South Sumatra Health Service released that the MMR in South Sumatra and Palembang reached 97 and 14 people respectively. As an effort to reduce the MMR, the Sustainable Development Goals (SDG's) set a global MMR target in 2030 of less than 70 per 100,000 live births. This final project report aims to provide support in reducing MMR by providing Continuity of Care (COC) for Mrs. "I" at PMB Ellna Palembang. This case study used primary and secondary data with notes on the development of SOAP and Varney's seven steps. The results obtained from Mrs. "I" are physiological pregnancy, labor with prolonged first stage, physiological postpartum and neonate, as well family planning assessment miscarriage because patient has not used the contraception. Conclusions from subjective and objective data were confirmed by the physiological assessment of pregnancy, postpartum and neonate, but the first stage of labor was prolonged. Plans and implementation are enforced according to the assessment. The advice given to practicing midwives is to maintain the quality of service for pregnant and postpartum women, as well as neonates.
