Abstract
WHO reported that the Maternal Mortality Rate (MMR) in 2017 was 462 per 100,000 live births compared to 11 per 100,000 live births. In 2019, the IMR (Infant Mortality Rate) was 2.5 million babies dying during the first month of life, 6,700 neonatal deaths every day. Furthermore, the MMR in Indonesia was 4,627 and the IMR was 18.21. It is also reported that in South Sumatra, the MMR are 97 people and IMR is 28,158, 72% of them occured during the neonatal period. In South Sumatra Province, the number of MMR in 2019 was 105 people and rose to 128 in 2020 and then rose more up to 131 in 2021. Medical record data at PMB "L" in the city of Palembang from 2021 – 2023 showed that the number of MMR and IMR was 0 people. This final report aims to provide Continuity of Care Midwifery Care for Mrs "R" at PMB 'L' Palembang starting from pregnancy, childbirth, newborn babies, postpartum and neonate with a charity spiritual approach. The method used SOAP recording which was carried out from June 4, 2023 to January 30 2024 starting from the period of pregnancy, childbirth, newborn babies, postpartum and neonates at PMB "L". Primary and secondary data were also used for data collection techniques. The results obtained from Mrs. "R" in pregnancy, postpartum and neonates are still within the physiological scope but in the first stage of labor, a referral is made with a diagnosis of Oligohydramion. The conclusion from the subjective and objective data was that the pregnancy assessment, KF and KN were within normal limits, while during delivery, a CS was performed at the "S" Hospital for the indication of megavesica. Plans and implementation are enforced according to assessment. The advice that the author gives to practice areas is to be able to maintain the quality of midwifery services.
