Abstract

Pregnancy and childbirth are physiological and natural things, although pathological things can occur in the process. The World Health Organization (WHO) reported that the Maternal Mortality Rate (MMR) in 2020 were 287,000 people worldwide. The data also showed that in 2022, the MMR reached 3,572 deaths in Indonesia. Furthermore, it was found out that in 2020, the MMR and Infant Mortality Rate (IMR) in Palembang was 14 and 18 people respectively. This report aims to inform the effort to reduce MMR and IMR by providing Continuity of Care (COC) Midwifery Care to Mrs. "R" based on “Spiritual Charitas” values. This final report is done from June 27, 2023 to February 14, 2024 in Independent Practice Midwife “E” from the first trimester of pregnancy until delivery, newborns, postpartum and neonates. The data collection technique used in this final report is primary data and secondary data, using SOAP Note (Subjective, Objective, Assessment, and Plan) method. The results showed that during pregnancy from the first to third trimesters, Mrs. "R" had no complications. It implied that the delivery was at term and progressed normally, while newborns, the postpartum period and neonates proceeded normally. The conclusion reveals that Continuity of Care (COC) Midwifery Care provided to Mrs. "R" during pregnancy, childbirth, newborns, postpartum and neonates occurred within normal limits and Mrs. "R" was cooperative while being given midwifery care and was able to apply the “Spiritual Charitas” values. It is hoped that midwives will be able to maintain and improve the quality of services provided to clients.
