Abstract
The Maternal Mortality Rate (MMR) in 2018 according to the World Health Organization (WHO) was 831/100,000 and in 2017 the Infant Mortality Rate (IMR) was 29/1000 KH. Data from the Health Office of Palembang City states that the Maternal Mortality Rate in 2018 was 4 people out of 26,837 KH and the Infant Mortality Rate in Palembang City was 24 people out of 29/1000.). The aim is to carry out Continuity Of Care Care for pregnant women, maternity, newborns, postpartum, neonates and family planning and implement SOAP documentation. Continuity Of Care (COC) Midwifery Care for Mrs. “E” is located at the Independent Midwife Ellna Palembang practice, starting from pregnancy, childbirth, Newborns (BBL), neonates, postpartum period, and Family Planning (KB). The method used is descriptive method, data collection uses primary data and secondary data observations, and documentation uses SOAP records (Subjective, Objective, Assessment, Plan) which was carried out from 07 April 2022-21 April 2022. The results obtained for Mrs. “E began from pregnancy, childbirth, newborns, 6 hours postpartum, and 6 days postpartum, 6 days neonates, and family planning within normal limits and still in the physiological space. The conclusion of the mother during pregnancy, childbirth, the puerperium, neonates, family planning did not find problems and was still within the scope of physiology. Suggestions for practice to maintain the quality of midwifery services.
