Abstract

Midwifery care continuity of care (COC) For Ny “F” at the independent practice of Midwife lismarini Palembang The World Health Organization (WHO) states that the high MMR in 2017 was 462 per 10,000 KH, in Indonesia according to (IDHS) in 2017 the MMR was still 15 per 100.00 KH. One of the efforts to reduce MMR and IMR is to carry out Continuity of Care (COC) midwifery care, namely continuous care from pregnancy to family planning as an effort to reduce MMR and IMR. The purpose of the final project report is to be able to carry out continuity of care (COC) midwifery care for Mrs "F" starting from pregnancy, childbirth, BBL, postpartum, neonates, and family planning at PMB Lismarini. This final project report provides midwifery continuity of care (COC) care for Mrs. 'F' at the Independent Practice Midwife from 01 December 2021 to 06 April 2022, starting from pregnancy, childbirth, newborns, postpartum neonates and family planning. The data collection technique used primary data and secondary data. The midwifery method used was a case study method with a Continuity of care (COC) approach to midwifery care and documentation of SOAP on Mrs "F" in pregnancy, childbirth, BBL, postpartum, neonates, and family planning. The conclusion based on the assessment data obtained on Mrs. "F" was diagnosed with pregnancy, childbirth, BBL, postpartum, neonates in a physiological scope, during family planning services. Mother said she wanted to discuss it first with her husband and would install contraception after 40 days. Plans and evaluations of midwifery care with continuity of care (COC) for Mrs. "F" during pregnancy, childbirth, BBL, postpartum, neonates, and family planning have been carried out in accordance with the established assessment. Advice given to patients is expected to always maintain their own health and the baby.
