Abstract

World Health Organization (WHO) reported that there were 295,000 of Maternal Mortality Rate (MMR) and 6,700 of Infant Mortality Rate (IMR) in the world in 2019. The Health Department of Palembang City stated that in 2019, the number of MMR was 4 per 26,837 Live Births (LB). Meanwhile, 24 IMR cases comprising mortality of 18 neonates (0 to 28 days) and 6 infants (29 to 11 months).The aim of this Continuity of Care (COC) is to prevent the MMR and IMR. The COC to Mrs. “S” was taken place at the Midwife Ellna’s Independent Practice Palembang. The COC was conducted from pregnancy, childbirth, newborns, neonates, postpartum, and family planning. This research used descriptive method and employed data collection technique through primary data, secondary data, observation, and documentation of SOAP note (Subjective, Objective, Assessment, Plan) which was carried out from November 18, 2021 to February 4, 2022. The results of COC to Mrs. “S” revealed that the pregnancy, childbirth, newborns, neonates, six - hour postpartum, six - day postpartum, six - day neonate and Family Planning were in normal limit and physiological scope. In conclusion, the mother did not have any health issuesduring the pregnancy, childbirth, the puerperium, neonates, and family planning. The condition was also still in the physiological scope. It is suggested that the Independent Midwifery Practice to maintain the midwifery service quality.
