Abstract
According to WHO, the number of Maternal Mortality Rate (MMR) was 462/100,000 LB in 2017. The Sustainable Development Goals (SDGs) states that it is expected that the number of MMR will be less than 70/100,000 LB. In Indonesia, the number of MMR was 4,627 in 2020 and 4,221 in 2019 with the Infant Mortality Rate (IMR) was 28,158/1,000 Live Births (LB). In South Sumatera, the number of MMR was 199 and the number of IMR was 51 in 2018. In Palembang, the number of MMR was 7 and the number of IMR was 29 in 2017. From October 2021 to January 2022, it was not found any maternal mortality or infant mortality at Midwife Lismarini’s Independent Midwifery Practice Palembang. Continuity of Care (COC) is one of sustainable midwifery caring efforts conducted by the midwife in order to decrease the MMR and IMR. The aim of this final report was to provide midwifery care of COC to twenty-five-year-old Mrs. “G” at Midwife Lismarini’s Independent Midwifery Practice Palembang. The COC was conducted from the pregnancy, labor, newborn baby, postpartum to neonate. The report of this study case used SOAP documentation and it was conducted at Midwife Lismarini’s Independent Midwifery Practice. The object was twenty-five-year-old Mrs. “G” and the COC was provided from the first, second, and third trimester of pregnancy, labor, newborn baby, postpartum, neonate, and family planning. It was conducted from October 1, 2021 to January 18 2022. Primary and secondary data were used to collect the data. In short, from the midwifery care to Mrs. “G”, it was figured out that the labor, newborn baby, postpartum, and neonate were in physiological scope and normal range. It is expected that this report is used as one of the references for conducting similar research. It is also expected that the writer completes the final report properly and will become a quality health worker.
