Abstract

Maternal Mortality Rate (MMR) is mortality in pregnancy period within 42 days and after pregnancy (WHO, 2021). A number of MMR causes are probably detected since the pregnancy period. The Midwifery Caring of Continuity of Care (COC) has effectively built emotional bonding with the client as “a friend” and decreased MMR cases. The aim of this final report is providing Midwifery Caring of COC to Mrs. “R” from pregnancy, labor, newborn baby, postpartum, neonate to family planning at Midwife Ellna’s Independent Midwifery Practice Palembang. The data were collected through primary and secondary data observation and the documentation implemented SOAP (Subjective, Objective, Assessment, Plan). This process was conducted from 21 April 2021 to 4 June 2021. The result revealed that the Midwifery Caring of COC to Mrs. “R” in ANC 1 and 3 was in physiological scope meanwhile breech presentation was found in ANC 2. However, from labor phase to neonate phase were occurring in physiological scope. In conclusion, the Midwifery Caring of COC has been provided to Mrs. “R” from pregnancy, labor, newborn baby, postpartum, to neonate phases and the client was cooperative. It is expected that the client is able to maintain health and take care of the baby through following guidance from the midwife so that the midwifery caring is properly conducted.
