Abstract
Data from the World Health Organization (WHO) Maternal Mortality Rate (MMR) is estimated at 462 per 100,000 in the world. In Indonesia, maternal deaths in 2015 were estimated at 305 per 1000 KH, the Palembang City Health Office stated that AKI in 2017 was 29 cases in 27,876 KH. The Infant Mortality Rate (IMR) in 2017 amounted to 29 cases. As one of the efforts to overcome AKI and IMR, Continuity Of Care care is needed to reduce AKI and IMR. This final project report provides Continuity Of Care Midwifery Care to Ny. “S” is 29 years old at the Independent Practice of Midwife Ellna Palembang 11 February – 20 May 2021, starting from pregnancy, childbirth, newborns, postpartum, neonates. Data collection techniques using primary and secondary data. Case study method used using SOAP developmental notes. The results obtained in Mrs. "S" starting from pregnancy with mild anemia, normal delivery, normal BBL, postpartum, and neonates, is still within the physiological scope. The conclusion that can be obtained is that the mother has mild anemia, normal delivery, normal BBL, postpartum, and normal neonates, still within the physiological scope. The author's suggestion is to improve self-skills in providing Continuity Of Care midwifery care services, and can be a source of learning in completing the Final Project Report and become qualified health workers in providing services.
