Abstract
The Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) remain critical public health challenges at both global and national levels. Reports from the World Health Organization (WHO) indicate approximately 260,000 maternal deaths in 2023 and 2.3 million neonatal deaths in 2022. To mitigate these rates, the implementation of Midwifery Continuity of Care (CoC), spanning pregnancy, labor, postpartum, and neonatal periods, is essential. This final report aims to provide comprehensive Midwifery Continuity of Care (CoC) for Mrs. “S” at Midwife “E” Independent Practice, Palembang, integrated with the Charitas Spirituality approach. The methodology employed was a case study utilizing the SOAP (Subjective, Objective, Assessment, and Plan) documentation framework, conducted from July 21, 2025, to March 06, 2026. Clinical results indicated that during pregnancy, Mrs. “S” experienced physiological discomforts, including first-trimester nausea and third-trimester lower back pain. The labor process proceeded normally, and both the postpartum and newborn outcomes remained within physiological limits. Furthermore, the integration of the Charitas Spirituality approach during the care process provided essential emotional support and enhanced maternal comfort and confidence from pregnancy through the postpartum period. In conclusion, the Midwifery Continuity of Care (CoC) for Mrs. “S” was successfully implemented, ensuring physiological progress and the absence of complications across the care continuum. It is recommended that the application of holistic CoC services be further enhanced to optimize maternal and infant health outcomes.
