Abstract

According to the World Health Organization (WHO), maternal mortality reached approximately 260,000 deaths in 2023, while infant mortality was recorded at 2.3 million in 2022. Efforts to reduce the Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) are implemented through continuous maternal and child health services, encompassing routine Antenatal Care (ANC), emergency detection and management, nutritional optimization for pregnant women, and the establishment of efficient referral systems. This final report aims to provide Midwifery Continuity of Care (CoC) for Mrs. “D”. The methodology utilized a case study approach with SOAP (Subjective, Objective, Assessment, and Plan) documentation, covering the stages of pregnancy, labor, newborn care, postpartum, and neonatal care. The period of care spanned from July 4, 2025, to January 23, 2026. Clinical results indicated that during the first trimester, Mrs. “D” presented with severe anemia (Hb 7.1 g/dL); however, hemoglobin levels were successfully restored and remained within normal limits during the second and third trimesters. Furthermore, the labor process, newborn care, postpartum, and neonatal periods progressed physiologically without complications. In conclusion, the client demonstrated a cooperative attitude throughout the assessment and care delivery. Despite the initial severe anemia in the first trimester, the condition was effectively managed, ensuring a normal physiological course across all subsequent stages of care. The planning and implementation of midwifery care were executed based on the client's specific needs while integrating the values of Charitas Spirituality. It is recommended that healthcare providers maintain and enhance the quality of comprehensive, professional, and client-centered midwifery services.
