Abstract
According to data from the World Health Organization (WHO), the Maternal Mortality Rate (MMR) in 2023 was 260,000 women who died during and after pregnancy and childbirth worldwide. The MMR in Indonesia in 2023 was 4,482. In 2022, the MMR in South Sumatra was 97, with an MMR of 64 per 100,000 live births. By 2030, the SDGs have a target of reducing global MMR to less than 70 per 100,000 births. The purpose of this care is for early detection of risk factors and complications that have the potential to cause MMR and IMR. Midwifery care of Continuity of Care for Mrs. "A" was carried out from June 19, 2025 - January 23, 2026 at PMB "L" Palembang starting from the first, second and third trimesters of pregnancy, childbirth, newborns, postpartum, and neonates. This case study was conducted with a midwifery management approach using the SOAP format and based on the spiritual values of charity. Data were obtained through interviews, observations, physical examinations, documentation studies, and literature studies. The results of the care showed that during the implementation of the case study, pathological conditions were found during labor so that the client required referral to a higher health care facility. Therefore, the care provided to Mrs. "A" includes care during the first, second, and third trimesters of pregnancy, the second and third postpartum periods (KF), and neonatal care through Neonatal Visits (KN) II and KN III. The implementation of Continuity of Care supports the provision of comprehensive and continuous services, optimally providing maternal and infant health care.
