Abstract

According to the World Health Organization (WHO), the Maternal Mortality Ratio (MMR) in 2023 stood at 346 per 100,000 live births, while the Infant Mortality Rate (IMR) in 2022 saw approximately 6,500 newborn deaths daily worldwide. In Indonesia, the MMR in 2023 was recorded at 4,482 deaths, and the IMR was 16.85 per 1,000 live births. In South Sumatra Province, there were 107 maternal deaths and 666 infant deaths. Based on medical records at PMB "L" in Palembang City for the 2023–2026 period, no cases of maternal or infant mortality were found (zero cases). This Final Project Report aims to provide Continuity of Care (CoC) midwifery services to Mrs. "R" at PMB "L" in Palembang, utilizing a *caritas* spirituality approach. The method employed was a case study, with documentation using the SOAP format. Care was administered from June 15, 2025, to January 21, 2026, covering the periods of pregnancy, childbirth, the newborn phase, the postpartum period, and neonatal care. Data collection involved primary data obtained through interviews, observations, physical examinations, and documentation, as well as secondary data obtained from Maternal and Child Health (KIA) booklets and medical records. The results of the care indicate that Mrs. "R"‟s condition progressed physiologically throughout the pregnancy, postpartum, newborn, and neonatal periods. However, a third-trimester ultrasound revealed a breech presentation, leading to a referral to "RA" Hospital, where the delivery was performed via Caesarean section (CS). Based on subjective and objective data, assessments conducted during pregnancy, the postpartum period (KF), and the neonatal period (KN) were within normal limits. Care planning and implementation were carried out in accordance with the established assessments. It is recommended that Independent Midwifery Practice(PMB) "L" continue to maintain and enhance the quality of midwifery care, ensuring it remains comprehensive, continuous, and client-centered.
