Abstract
Background: MCV is a red blood cell index parameter used to classify anaemia based on red blood cell size. MCV test results are influenced by pre-analytical factors, one of which is the headspace in the K₂EDTA tube. The headspace plays a role in maintaining the balance between the blood and anticoagulant and aids in sample homogenisation. Differences in the volume of blood introduced into the tube can alter the proportion of free air space and the blood-to-anticoagulant ratio, thereby affecting erythrocyte stability and the MCV. Objective : Determining the difference in MCV values between a free air space of more than 20% and a free air space of 20% using a 3 mL K2EDTA tube. Methods : A quantitative study with an analytical observational design and a cross-sectional approach. The study was conducted at the Haematology Laboratory of the Faculty of Health Sciences, Musi Charitas Catholic University, Palembang. The study sample consisted of 47 respondents aged 18–23 years, selected using a non-probability sampling technique based on Lemeshow’s formula. Data analysis was performed using a paired-sample t-test. Result : The average MCV for a 20% air gap is 87,489 fL and for a 20% air gap 87,391 fL. Conclusion : There is a significant difference in MCV test results between samples with a free air space of 20% and those with a free air space of >20%. Recommendation : Fill the K₂EDTA tube with the recommended volume of blood to ensure the accuracy of the MCV result. Further research is needed, varying the sample volume, headspace, time to analysis, type of EDTA anticoagulant, and different haematology analyser methods.
