Abstract

Background: Clinical Laboratory is a health laboratory that conducts clinical specimen examinations to obtain information about individual health, mainly to support disease diagnosis, treatment, and health recovery. Laboratory examinations consist of three phases: pre-analytical, analytical, and post-analytical. Several factors can affect MCH values, one of which is the air space inside the tube. This air space can be determined by the tube's volume capacity. If the blood volume filled is less than 75% of the total tube volume capacity, it will result in free air space exceeding 20%. Objective: To determine the difference in MCH values from blood samples collected in K2EDTA tubes of 1.5 mL and 3 mL volumes with variations in free air space greater than 20% and exactly 20%. Method: This study used an analytical observational design with a cross-sectional approach. The research was conducted in the Hematology Laboratory at UKMC. The subjects were 47 female students aged 18-23 years, selected by non-probability sampling using purposive sampling technique. MCH measurement was performed using impedance method based on counting and measuring cell volume. Data were analyzed using the Wilcoxon Signed Rank Test with a 95% confidence level. Results: The average MCH value for the 1.5 mL volume was 27.57 pg, while for the 3 mL volume, it was 27.53 pg. Conclusion: There is no difference in MCH values between the 1.5 mL sample with free air space variation greater than 20% and the 3 mL sample with free air space variation of 20%. Suggestion: MCH measurement can still be performed using blood samples with a volume of 1.5 mL, even when the free air space inside the tube exceeds 20% of the tube's capacity. Future studies are recommended to evaluate the use of alternative EDTA vacutainer tubes, such as Na₂EDTA and K₂EDTA, with blood sample volumes of less than 1.5 mL and free air space exceeding 20%, to further investigate their effects on hematological parameters.
