Abstract

Background: The Mean Corpuscular Hemoglobin Concentration (MCHC) test is a red blood cell index parameter that reflects the average concentration of hemoglobin in one deciliter of red blood cells. MCHC values can be influenced by inaccuracies in blood volume and oxygenation levels. The pre-analytical phase accounts for approximately 61% of total laboratory errors, one of which is an inappropriate blood volume in K₂EDTA vacuum tubes. An inappropriate blood volume can alter the blood-to-anticoagulant ratio and increase the headspace within the tube, potentially affecting hematological test results. Objective: To determine the differences in MCHC values between 1.5 mL and 3 mL blood samples collected in 3 mL K₂EDTA tubes, where the 1.5 mL sample contains more than 20% headspace. Methods: This study was an analytical observational study with a cross-sectional design. The research was conducted at the Hematology Laboratory, Faculty of Health Sciences, Musi Charitas Catholic University, Palembang. A total of 47 participants aged 18–23 years were included in the study. Samples were selected using a non-probability sampling technique. Data were analyzed using the Wilcoxon signed-rank test at a 95% confidence level. Result: The mean MCHC value was 31.57 g/dL for the 1.5 mL samples and 31.59 g/dL for the 3 mL samples. Conclusion: There was no significant difference in MCHC values between samples with a volume of 1.5 mL (free air space >20%) and samples with a volume of 3 mL, as indicated by the statistical analysis, with a p-value 0.558 (p > 0.05). Suggestion: Further research is recommended using tubes containing K₃EDTA or Na₂EDTA anticoagulants to evaluate the effect of sample volume variations on MCHC values.
