Abstract
The Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) remain pivotal indicators for evaluating public health standards. On a global scale, maternal fatalities have reached approximately 260,000 cases, with an IMR recorded at 17 per 1,000 live births. Within the Indonesian context, 2024 data reported 4,150 maternal deaths and 31,393 infant deaths. To mitigate these rates, the implementation of Continuity of Care (CoC) serves as a fundamental midwifery strategy. Accordingly, this study aimed to provide comprehensive CoC for Mrs. “S” at Midwife “L” Independent Practice Palembang, underpinned by Charitas spirituality values. Using a case study design, midwifery care was documented using the SOAP (Subjective, Objective, Assessment, Plan) method for Mrs. “S” (G2P1A0) from June 16, 2025, to January 4, 2026. This care spanned the antenatal, intranatal, postnatal, new-born, and neonatal periods, with data collected from both primary and secondary sources. The results indicated that the mother’s condition throughout pregnancy, delivery, and the postpartum period, as well as the status of the new-born and neonate, remained within physiological (normal) limits. Based on the clinical data, the assessments for all stages were confirmed as normal, with plans and implementations tailored to the patient's specific needs. This study concludes that comprehensive and continuous midwifery care effectively supports maternal and infant health from pregnancy through the neonatal stage. It is hoped that this report will serve as a clinical reference to maintain and enhance the quality of midwifery services.
