Abstract
World Health Organization (2025) reported that the global maternal mortality ratio (MMR) was 197 per 100,000 live births, while the neonatal mortality rate (NMR) was 17.2 per 1,000 live births. In Indonesia, the Ministry of Health (2025) reported a maternal mortality ratio of 183 per 100,000 live births and an infant mortality rate (IMR) of 16.85 per 1,000 live births. These figures underscore the importance of providing comprehensive and continuous maternal and new-born care to improve health outcomes. This final project aimed to implement Midwifery Continuity of Care (CoC) for Mrs. “M,” covering pregnancy, childbirth, the postnatal period, new-born care, and neonatal care through a midwifery management approach grounded in Charitas spirituality. The care was provided at an Independent Midwifery Practice in Palembang from July 11, 2025, to January 27, 2026. Data were collected from both primary and secondary sources and analysed using a midwifery management framework based on the principles of Charitas spirituality. The findings showed that Mrs. “M” experienced a normal pregnancy until the late third trimester, when she developed preeclampsia. Early identification of this complication enabled timely referral and delivery by caesarean section. Both the mother's condition during the first six days of the postnatal period and the new-born’s condition during the first six days of the neonatal period were within normal limits, with no complications observed. In conclusion, the implementation of Midwifery Continuity of Care facilitated the early detection and appropriate management of preeclampsia, contributing to favourable maternal and neonatal outcomes. It is recommended that independent midwifery practices continue to strengthen the quality of comprehensive and continuous midwifery services to support improved maternal and neonatal health.
