Abstract

CV Sukses Jaya Mandiri is a company engaged in rice production and distribution. In its production process, the company still faces quality problems in the form of damaged or substandard rice, which may reduce customer satisfaction and affect the company’s reputation. Therefore, an analysis is required to identify the factors causing failures in the production process and to determine improvement priorities. This study applies the Failure Mode and Effect Analysis (FMEA) method to identify types of failures, analyze their causes and effects, and determine the Risk Priority Number (RPN). In addition, a Fishbone Diagram is used to systematically identify the root causes of the problems. The results show that the dominant factors causing rice defects originate from suboptimal drying, storage, and packaging processes. Based on the highest RPN values, the company is recommended to improve warehouse temperature and humidity control, conduct regular machine maintenance, and strengthen quality supervision to reduce defect rates and improve the quality of the rice produced.
