Abstract

In 2023, it is reported that the Maternal Mortality Rate (MMR) reached 260.000 women. Furthermore, in 2022 the Infant Mortality Rate (IMR) was 6.500. The Ministry of Health recorded that maternal mortality in 2022 reached 230 per 100.000 live birth (the rate was 20.882). The profile of the South Sumatra Provincial Health Service in 2020 described that the MMR increased to 128 and the IMR increased to 282. In 2022, the MMR decreased again to 4 people, while the number of maternal infant deaths decreased again to 16 people. This final project report aims to provide midwifery care with Continuity of Care to Mrs. "V" by implementing the Charitas Spirituality approach. This Final Assignment Report was conducted from March 25, 2025 to June 13, 2025 at Independent Practice Midwife “E” Palembang starting from the second trimester of pregnancy, childbirth, newborns, postpartum, and neonates. This case study method used SOAP approach which includes data from Subjective, Objective, Assessment, and Plan. During a normal pregnancy, Mrs. V complained of itching on her arm (physiologically). Finally the Independent Practice is recommended to maintain and improve the quality of care for pregnant women, women giving birth, postpartum mothers, newborns, and neonates.
