Abstract

According to the World Health Organization (WHO, 2023), maternal mortality rates in vulnerable countries remain alarmingly high, averaging 551 per 100,000 live births, or more than twice the global average. In Indonesia, maternal deaths reached 3,572 in 2022, with 18,281 neonatal deaths recorded during the same year. In South Sumatra, the Maternal Mortality Rate (MMR) was 97 cases and the Infant Mortality Rate (IMR) was 430, while in Palembang City the figures were lower, with an MMR of 4 and an IMR of 0.9 per 1,000 live births. This study aimed to contribute to the reduction of maternal and neonatal mortality through the implementation of midwifery continuity of care (CoC) services for Mrs. “Y,” grounded in the spirituality of Charitas. Conducted from December 10, 2024, to June 21, 2025, at Midwife “E” Independent Practice in Palembang, this case study encompassed comprehensive care across the continuum of pregnancy, childbirth, newborn care, postpartum, and neonatal stages, employing the SOAP framework (Subjective, Objective, Assessment, and Plan). The findings indicated that Mrs. “Y” experienced a normal pregnancy, with mild physiological dizziness during the second trimester, followed by normal labor, delivery, and newborn outcomes, and postpartum recovery within normal physiological parameters at Hospital “A.” The study underscores the importance of maintaining and continuously improving the quality of midwifery care for pregnant, laboring, and postpartum women, as well as for newborns and neonates, to optimize maternal and child health outcomes.
