Abstract

Complications during pregnancy, childbirth, the immediate newborn period, postpartum, and the neonatal phase contribute significantly to maternal and infant mortality rates. According to the World Health Organization (WHO), the Maternal Mortality Rate (MMR) in 2023 was 346 per 100,000 live births, while the Infant Mortality Rate (IMR) in 2022 reached 6,500 newborn deaths per day. In Indonesia, the MMR was recorded at 4,482 and the IMR at 16.85. In South Sumatra, the MMR was 107 and the IMR 666. However, based on medical records at Midwife "L" Independent Practice in Palembang from 2022 to 2024, there were zero cases of maternal or infant mortality. This final report aims to present Midwifery Continuity of Care (CoC) for Mrs. “D” at Midwife “L” Independent Practice, Palembang, covering the continuum of care from pregnancy, childbirth, immediate newborn care, postpartum, to neonatal periods using a CHARITAS-based spiritual approach. The method used is a case study applying midwifery management through the SOAP documentation method (Subjective, Objective, Assessment, and Plan), conducted from July 12, 2024, to February 2, 2025. Data were collected through both primary and secondary sources. The results showed that Mrs. “D” experienced physiological conditions during pregnancy, postpartum, and the neonatal period; however, during the first stage of labor, a referral was made due to a transverse fetal position. Consequently, a cesarean section was performed at Hospital “R.” Postpartum and neonatal care were within normal limits. This case emphasizes the importance of maintaining high-quality midwifery services at the practice site to ensure maternal and newborn well-being.
