Abstract
The World Health Organization (WHO) reported that the global maternal mortality rate (MMR) was 197 per 100,000 live births in 2023, while the global infant mortality rate (IMR) reached 2.3 million deaths in 2022. In Indonesia, maternal and infant mortality rates remain alarmingly high, with an MMR of 4,482 per 100,000 live births and a total of 27,530 infant deaths reported in 2023. In the province of South Sumatra, the MMR in 2023 was recorded at 105 cases, and the IMR stood at 2.4 per 1,000 live births. Continuity of Care (CoC) in midwifery refers to comprehensive and continuous care provided throughout pregnancy, childbirth, the postpartum period, and newborn care. This approach aims to prevent complications during these critical periods and is expected to contribute to reducing the persistently high rates of maternal and infant mortality in Indonesia. This case study documents the midwifery CoC provided to Mrs. “D” at the Midwife “L” Independent Practice in Palembang, from August 18, 2024, to March 4, 2025. The care covered all phases: early pregnancy, labor and delivery, postpartum, and the neonatal period. Data collection utilized both primary and secondary sources. The case study design was based on the SOAP (Subjective, Objective, Assessment, and Plan) documentation format. The findings showed that Mrs. “D” experienced mild anemia during the second trimester, which had resolved by the third trimester with hemoglobin levels returning to normal. Labor proceeded normally, the newborn was healthy, the postpartum period was uneventful, and neonatal development remained within normal limits. In conclusion, the care provided to Mrs. “D” involved a full-term pregnancy, normal delivery, a healthy newborn, a stable postpartum period, and a neonate developing within the expected range. It is recommended that the clinical practice site continue to maintain and enhance the quality of its midwifery services.
