Abstract

World Healh Organization (WHO) reported that approximately 260,000 women died during and after pregnancy and childbirth in 2023. In Indonesia, the maternal mortality rate (MMR) reached 4,482, while the Infant Mortality Rate (IMR) was 27.530 where the majority of deaths occur in the neonatal period (0-28 days). In addition, it is also informed that the maternal and infant mortality were 16 per 30.109 live birth and 104 per 30.109 live birth descriptively. This final project report aims to provide midwifery care with Continuity of Care to Mrs. "Y" during pregnancy, childbirth, newborn, postpartum and neonate with the Charitas Spirituality approach. Case study approach was implemented as the method in this study, using primary data techniques and secondary data using data collection instruments (SOAP notes). This Final Assignment Report was conducted from July 25, 2024 to January 28, 2025 at Independent Practice Midwife “E” Palembang starting from the first trimester of pregnancy, childbirth, newborns, postpartum, and neonates. The data collection techniques involved both primary and secondary data, using SOAP notes. The results implied that Mrs. "Y" ‘s pregnancy did not experience any complications, delivery at full term gestational age and proceeded normally, the baby was born without any congenital abnormalities, the postpartum period was normal, and the neonate proceeded normally. It is concluded that the Continuity of Care carried out on Mrs. "Y" during pregnancy, childbirth, newborns, postpartum and neonates took place within normal limits and Mrs. "Y" was cooperative during midwifery care and was able to apply the values of Charitas Spirituality. Recommendations are adressed to the midwives to maintain and improve the quality of services provided to clients.
