Abstract

According to the World Health Organization (WHO), maternal mortality (MMR) accounted for approximately 260,000 deaths among women during pregnancy and childbirth, while infant mortality (IMR) reached 29,945 cases in 2023. In Indonesia, maternal mortality was recorded at 4,482 cases and infant mortality at 34,226 cases in the same year. In Palembang City, 2023 data showed 105 cases of maternal mortality and 370 cases of infant mortality. The purpose of this report is to provide midwifery continuity of care for Mrs. “D” during pregnancy, childbirth, newborn care, postpartum, and neonatal periods, based on the spirituality of Charitas. This case study, as part of the final project report, was conducted from November 10, 2024, to April 28, 2025, at Midwife “L” Independent Practice Palembang, encompassing the first trimester of pregnancy through childbirth, newborn, postpartum, and neonatal care. Data were collected using both primary and secondary sources, documented with SOAP (Subjective, Objective, Assessment, and Plan). The results showed that Mrs. “D” experienced normal pregnancy from the first to the third trimester. Childbirth occurred at term through referral with diagnoses of premature rupture of membranes (PROM) and oligohydramnios. The postpartum period and neonatal care proceeded normally. The study concludes that continuity of care midwifery services provided to Mrs. “D” throughout pregnancy, childbirth, newborn, postpartum, and neonatal care were successfully delivered, with the client remaining cooperative and able to apply the values of charitas spirituality. The author recommends that midwives continue to maintain and enhance the quality of care provided to clients.
