Abstract
According to the World Health Organization (WHO), the global Maternal Mortality Rate (MMR) in 2023 was 303,000 deaths, while the Infant Mortality Rate (IMR) reached 2.3 million. In Indonesia, the MMR was recorded at 4,482 deaths and the IMR at 34,226. In South Sumatra, the MMR was 105 and the IMR 430, with Palembang reporting 64 maternal deaths and an IMR of 16 per 1,000 live births. In contrast, both MMR and IMR at Midwife “L” Independent Practice were recorded as zero. One effective strategy to reduce maternal and infant mortality is the implementation of midwifery Continuity of Care (CoC) during pregnancy, childbirth, postpartum, and the neonatal period. This final report presents the provision of CoC for Mrs. “N” at the independent practice in Palembang, under the CHARITAS values. The care process, documented using the SOAP method (Subjective, Objective, Assessment, and Plan), was conducted from July 6, 2024, to January 8, 2025, and covered all stages from pregnancy to neonatal care. Data were obtained through both primary and secondary sources. The results showed that Mrs. “N” experienced physiological conditions throughout pregnancy, childbirth, newborn care, and the neonatal period. Based on subjective and objective data, assessments indicated normal conditions across all stages, and the mother demonstrated cooperative behavior. The care plan and implementation were carried out in accordance with the assessment results. It is recommended that midwives continue to apply up-to-date health knowledge and enhance the quality of midwifery care in accordance with established standards to improve maternal and child health outcomes.
