Abstract

Advances in information technology has a great impact on human life, either institutions or companies use information technology. Information system can provide solutions from problems, so that activities will become more maximal and efficient. In the manufacture of this system, waterfall model as a method consists of Definition of Requirements, System and Software, Implementation and Unit Testing, Integration and Testing System, and Operation and Maintenance, and using PHP and MySQL tools as its database.
