Abstract
Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) are still a global problem. WHO recorded around 260,000 maternal deaths in 2023 and 2.3 million neonatal deaths in 2022. In an effort to reduce these numbers, the Sustainable Development Goals (SDGs) set a target of MMR <70 per 100,000 live births and IMR <12 per 1,000 live births. One of the strategic efforts that can be done is through Continuity of Care (COC). This final project report aims to provide COC Midwifery Care to Mrs. “A” at Independent Practice Midwife “K” Palembang with the Charitas Spirituality approach. The method used is a case study with the SOAP approach (Subjective, Objective, Assessment, and Plan) carried out on Mrs. “A” from July 16, 2024 to January 31, 2025. Primary and secondary data were used in data collection. The results of the care showed that complaints during the first trimester of pregnancy were nausea, and in the third trimester were back pain and vaginal discharge, but were still within physiological limits. Labor took place quickly due to good maternal support and activity. The postpartum period and the baby's condition were within the normal range. The conclusion of the final report revealed that the subjective and objective data assessment did not find any pathological problems and the mother was cooperative during the assessment process. The assessment was established based on the results of the subjective and objective data assessment. Plans and implementation were carried out according to needs and care by applying the values of Charitas spirituality. Recommendations are expected to improve the quality of services, especially in the provision of midwifery care with Continuity of Care.
