Abstract

According to the World Health Organization (WHO), Maternal Mortality Rate (MMR) in 2023 was recorded at 260,000 deaths, while the Infant Mortality Rate (IMR) in 2022 reached approximately 6,500 deaths. In Indonesia, the MMR in 2021 was 7,389 deaths, with South Sumatra Province reporting 97 maternal deaths in 2022, and Palembang City recording 14 maternal deaths in 2020. In contrast, no maternal or infant deaths were reported at Midwife “E” independent practice in Palembang. Midwives play a vital role in reducing MMR and IMR through the provision of midwifery Continuity of Care (CoC) , which include regular antenatal care, safe delivery practices, adequate postpartum care, and proper monitoring of newborn health. This study aimed at providing midwifery CoC for Mrs. “W”, covering the stages of pregnancy, childbirth, newborn care, postpartum, and neonatal periods by applying midwifery care management integrated with the Charitas spirituality at Midwife “E” independent practice. This report employed primary and secondary data collection techniques using the SOAP (Subjective, Objective, Assessment, Plan) format of midwifery care documentation. The results showed that Mrs. “W” experienced emesis gravidarum during pregnancy, umbilical cord entanglement during labor, and that both the newborn and the postpartum period remained within physiological norms. In conclusion, the case study revealed that Mrs. “W” underwent a normal and physiologically appropriate course during pregnancy, childbirth, newborn, postpartum, and neonatal care. It is recommended that midwives continue to uphold and enhance the quality of midwifery CoC for pregnant women, mothers in labor, newborns, postpartum mothers, and neonates.
